The Exclusive icon is still on this item, but you can buy it in a bazaar. I don't know what exactly the deal is. Not sure if you can sell it to a npc or put it up in the AH.

You get these from a Moogle Rod, the rods have infinite uses, i don't know the re-use time.
I was lucky enough to have a friend with the rod, said she went to a FFXI roadshow and got it for going.

Yes these do come from Moogle Rods. I Believe that the reuse time on the item is 20hours with infinite uses. I believe it lasts 3 hours (I have two but have yet to eat then to comfirm this).
