I did the Pitchfork quest in San D'Oria but I'm posting what is supposed to be the way to get it in all three nations. The only one I can vouch for, though, is Sandy.

Make a party of two people. One person gets a Ghost costume (I got mine from Phamelise) and the other a Skeleton costume (my amoureux got his from Corua). Approach the huge Bomb decoration canopy-thingie near the MH in Southern Sandy and when you near it you'll both receive the Pitchfork.

In Bastok: -But I didn't do this personally- One person wears a Ghost costume and the other a Hound costume and they approach the Bomb decoration near Harmodios' Music Shop.

In Windurst -Didn't do this personally- One person wears a Skeleton costume and the other a Hound costume and they approach the Bomb decoration near the Culinary Guild.

To get the Pitchfork +1, you go to another nation and, with the first (NQ) Pitchfork equipped, get a costume from the roaming Trick NPC who would normally turn you into your nation's enemy beastman race Orc, Quadav, or Yagudo). You will instead become a Goblin.

My two-person San D'Orian party went to Windurst and got our Goblin costumes from a Trick Specter. We then approached the Bomb decoration near the Aurastery (K-6) to each get our Pitchfork +1.

(The following is from other sources that I can't vouch for, but it's likely true. Also, there are probably other roaming Trick NPCs besides the ones posted who will give you the costumes you need.)

A San D'Orian can get a Goblin costume in Bastok from a Trick Spirit. If you're San D'Orian and got your first Pitchfork in another nation, you get a Goblin costume from a Trick Shadow in San D'Oria to get your +1 there.

If you're Bastokan and want to get the +1 in Windurst, you get a Goblin costume from a Trick Shade. In San D'Oria, a Bostokan gets their Goblin costume from a Trick Bones. A Bostokan who got the first Pitchfork in another nation and wants to get the +1 in Bastok gets a Goblin costume from a Trick Phantom.

If you're Windurstian and want to get your +1 in San D'Oria, you get a Goblin costume from a Trick Shade. In Bastok, a Windurstian gets a Goblin costume from a Trick Skeleton. A Windurstian who got the first Pitchfork in another nation and wants to get the +1 in Windurst can get a Goblin costume from a Trick Ghost.

Again, all but the way for a San D'Orian to get the NQ Pitchfork in San D'Oria and the +1 in Windurst is from other sources that I didn't verify.

I have this and when I first got it, I thought maybe I was moving faster, but maybe it was just the excitment of getting it finally. I put it on now and I swear I don't move any faster. I also don't see anyone else running around with one on...is that maybe because they don't work? Is there some special trick to making it work?

The Pitchfork and Pitchfork +1 are a pain if you dont know the correct combinations. I dont know all the combinations but here is what I have so far.. and how you do it. if you want to update or add in go for it.

I am pretty sure the Costume Combination either changes for each city or for what ever city your character happens to be from..

If you and your partner are Sandorian and in Sandy then this combonation should work. Wolf/Ghoul (Ghoul is the guy looking costume) the Trick NPC that will give you this costume is The Trick Phantom (Ghoul) and Trick Skeleton (Wolf) run to each of the Bomb Decorations (2 in the fountain area, 1 in the area by Gertrude, one in the area to Port as you enter) one of them will give you the Pitchfork. To get the Pitchfork +1 you need to go to Bastok and trade a sweet to the Trick Spirit until it turns you to a gob.. once you and your partner have become Gob's then visit the decorations in the city to get the +1

Bastok Residents starting in Bastok...
Costume Combination Ghost / Wolf Trick NPC's to get costumes from.. are the Trick Ghost and the Trick Spector, Visit the Bomb Decoratins (there are 4 total) until you get the Pitchfork.. Next go to Sandy (I say that because I know the Trick NPC in Sandy required to finish this) Trade sweet to the Trick Bones to become Gob and visit Decorations there until you have the Pitchfork +1..

I do not know the costumes or NPC's needed in Windy yet.. I will update as I go along.

This post is mostly correct in obtaining the pitchfork and pitchfork +1 for Sandy residents.

Helpful Hints:

It is not required for you to visit each of the Bomb Decorations only one of them.

To obtain the gob costume in Bastok you MUST trade sweets to the "Trick Spirt" (bogy)

The item states clearly: "Costume: Increases Movement Speed" this means, and implys your movement speed is only increased while wearing a costume.

Also, NPC's do now take the pitchfork and pitchfork +1 but for a hefty fee. To retrieve each item costs 500gil.

'Somepage' has a list of what mob gives what costume for each city.

The pitchfork comes from the decoration in Windy Waters directly North from the MH.

We got the regular in North Sandy as wolf & shade from the decoration by the smithing guild.

Both of us were residents of Sandy so when we went to get the +1 we had to find 'trick bones' to get our gob costume.

to get this, find the npc that changes you into a goblin. That NPC is linked your home nation, that is, it will turn anyone of another nation into your country's main beastmen (yags for windy, orcs for sandy and quads for bastok). Once you find this NPC party with another player and have them find the npc for them. Once your both Goblins run to the Bomb decorations, theres 4 you need to visit. When you reach the last Bomb decoration, you will automatically receive the Pitchfork +1. Also when you trade the sweets to the NPC make sure your wearing the NQ pitchfork.

While each of the nations three cities do indeed have NPC's that store event items, the do not accept the Pitchfork or Pitchfork +1 to store for whatever reason. They also don't accept the Red Sash or Dash Sash. Hopefully SE will adjust this sometime soon, because like Gevlorien, my mog safe and storage is about to explode. -_-;
