Hi all,
I've just completed the mission Save the Children and noticed I don't have the title 'The Fodderchief Flayer' I've visited the NPC who allow us change our titles, but its not listed there. Have I missed something? I've reached rank 2 also.
Any advice?

Edit: Ah am I right in thinking that certain NPC title changes will only change certain titles? I've just been reading http://wiki.ffxiclopedia.org/wiki/Titles and noticed Yulon-Polon in Selbina will change the title I'm looking for.

Yes, each title is only given by a certain npc.
For instance, for the Warrior of the Crystal title (that is needed to unlock the crag -> sky warps), you specifically must see the title bard in Tavnazian Safehold for it. Not really a concern for you yet if you're a new player, but that is one of the (extremely few) examples in the game where it actually requires you to have a specific current title in order to do something. In most cases simply having the title unlocked is enough, but in this particular case you must make it your current title, meaning you have to make use of the title bard if your title has already changed to something else (which it usually has by that point)

I've just returned to the game after a few breaks. I've started a Warrior on Odin called Flamecrest.
FFXI has always drags me back ever since playing back in 2005.
