does exceptional result mean random or if you were lvl 75 goldsmithing and you made a copper hairpin it would be +1?

Exceptional Result means that while making the basic version of an item (in this case, the Brass Subligar) you got lucky and made a High Quality version (the Brass Subligar +1). As you go up in skill, getting HQ synths on lower skill items does get easier, but it is still not a guaranteed thing.
