To get this quest I needed the Jeuno fame :
"Your name is mentioned quite often"
Mendi is the NPC in lower jeuno, who tells fame, he is in the middle under an umbrella at the seaside.

In-Game TextDescriptionWhen you enter Neptune's Spire in Lower Jeuno, you will see a Tarutaru NPC named Sutarara in a room, along with Ghebi Damomohe behind the counter. Sutarara talks about the Tenshodo being "right under your nose." The Neptune's Spire is actually the Tenshodo Headquarters. To gain entrance, you need to select the third, blank line when speaking with Ghebi behind the counter. She will say that you need either a Tenshodo Invite or an Application Form. Head to Port Bastok and talk to Jabbar in Warehouse #2 across from the Airship Agency to receive the Application Form, then head back to Ghebi in Neptune's Spire. You will gain entrance to the Tenshodo trade ring, a Tenshodo Invitation, and more importantly, finish the quest. after i have given the girl my Tenshodo Invitation and enter the door what schould i do with the Tenshodo Invitation sell them or do i need it later?

Yes, you can either give it to somebody or sell it at the AH. It (obviously) allows somebody who hasnt done the quest to become a member

I can't select the third option too. I think it is because of my fame? but... Which area's fame do i need? Jeuno? bastok ? sandoria or windurst ? I have no idea. I think i will just do any quest from any country to get my fame up.

If your a NIN then joining the Tenshodo will help you out greatly. You can buy your scrolls here, your ninjutsu powders and the lvl 18 shurikens, all for less than the AH prices. They have the first 3 sets of the MNK/SAM/NIN armor, regular, cotton and soil (ex. sitabaki, cotton sitabaki, soil sitabaki). Plus, you get to see Aldo who is the leader of the Tenshodo and the little boy in the opening movie the game.

You dont have to select the third blank, i didnt, just trade the invite to her (i got from AH). You get in to the ring, and the title, really simple

You do not get a tenshodo ring, you get a key item that is like the "Tenshodo Memebership Pass" or something like that.

If you really want to get the quest over quick, just buy the Tenshodo Invite from the Auction house, and resell the one you get when you finish the quest.
Oh, and the correct title is Tenshodo Member.

I spoke with the npc in the 2nd room on the left (upon entering), then spoke with the npc at the counter and scrolled to the "hidden" option below "I'm fine". Now I just have to hoof it back to Bastok.

I have the same problem, lvl 24 BLM, cannot select "3rd blank" nor can I get NPCs in Bastok Warehouse #1 to give me an application form.

Level 24 Thief, and I cannot get the third blank option from the NPC behind the counter either. I was able to talk to the NPC in the room though. Can someone please explain to us, why not??

I am a lvl 19 war and could select it. Then bought the invite at the auction house.

Neptune's Spire is an inn. Inns have rooms. Thus, the NPC is in a room in the inn.

Im I level 25 Thief and I cannot choose the 3rd option either.

They sell stuff that you usually cannot get at other vendors, like SAM/NIN/MNK only armor and equip. They are a bit expensive, but it's sometimes better than the prices at AH.
