To spawn Lord Vyemm, its an event of 4 PH, killed in order. The next one spawns upon the death of the previous one.

Upon the death of the vicious leader, Lord Vyemm spawns. Lord Vyemm paths around in a circle on the lower level at the entrance 4-way by illustrated in the red circle on the map image.

Also, the spawns 2-4 spawn in other rooms on this floor going clockwise from the first one (which is in the NE), so
1) vile defender (NE) then after 6 minutes or so
1.5) vicious defender (NE) then immediately
2) vicious scholar (NW) then immediately
3) vicious warrior (SW) then immediately
4) vicious leader (SE) then immediately
Lord Vyemm outside the rooms, roams around the circle.
