usually, when you craft an item with a good success rate, there are good chances that similar level items will have similar success rates
at smithing level 13.4, you should be making iron ingots, and bronze stuff...
you make iron equipment past the smithing level 20 cap, and iron equipment is level 24 to equip
since this axe is level 34, im guessing you need a similar smithing level to craft it (unless you dont mind a ridiculously high fail rate)

From the looks of the ingredients and the level of the weapon I'm pretty sure it's a +30 skill item. Considering other items though, I'd say it's higher than that even.
