The level listed is the skill level at which you will cease to receive skill-ups by crafting the item.
I dont think the (4) means you can start at lvl 4 gold.. i think that's what lvl of goldsmithing where you stop getting crafting points.. not sure, but almost
I Smithed this at Lv.0. So, you don't Smith it at Lv.4 Smithing skills.
